What is Tympanoplasty?

Tympanoplasty is a surgical procedure performed to repair a perforated (hole) or damaged eardrum (tympanic membrane) and, in some cases, reconstruct or repair the small bones of the middle ear (ossicles). It aims to restore hearing, prevent recurrent ear infections, and improve ear function.

Symptoms

Ear pain or discomfort
Persistent hole in the eardrum not healing on its own
Fluid discharge (pus/mucus) from the ear
Repeated ear infections (chronic otitis media)
Difficulty swimming or showering due to water entering the middle ear
Persistent hole in the eardrum not healing on its own

Causes

Acoustic trauma (sudden loud noise)
Previous ear surgeries
Traumatic injury (cotton swab, foreign object, sudden pressure changes, blast injuries)
Chronic middle ear infections
Untreated ear infections leading to perforation

Types of Tympanoplasty

Advanced cases requiring fenestration procedures
Eardrum repair with partial ossicle involvement.
Advance Cases
Eardrum repair with partial ossicle involvement.
Small eardrum graft with ossicle removal except stapes footplate.

Tympanoplasty Diagnosis

Physical examination (Otoscopy)
Hearing test (Audiometry)
Tympanometry
CT scan (if needed)

Tympanoplasty (Repair of the Eardrum)

01
Type I Tympanoplasty (Myringoplasty)

This is the most common procedure.
The surgeon repairs the perforated eardrum using a graft (usually from the patient’s own tissue like fascia or cartilage). It restores hearing and prevents repeated ear infections. Suitable when middle ear bones are intact.

02
Type II Tympanoplasty

Performed when the malleus (one of the middle ear bones) is damaged. The graft is placed onto the incus or remnant malleus to restore the sound conduction pathway.

03
Type III Tympanoplasty

Done when malleus and incus are damaged. The graft is placed directly on the stapes bone to re-establish hearing.

04
Type IV Tympanoplasty

Used in more advanced disease when only the stapes footplate remains. The graft is shaped to protect the inner ear while improving sound transmission.

05
Type V Tympanoplasty

A rare procedure used when the stapes footplate is also affected. The surgeon creates a new sound pathway to improve hearing.

06
Endoscopic Tympanoplasty

A minimally invasive technique using an endoscope through the ear canal.

Benefits include no external cuts, faster recovery, less pain, and a clearer view of the middle ear.

07
Cartilage Tympanoplasty

In cases of large perforations or repeated failures, cartilage grafts are used for stronger, long-lasting repair.
Ideal for patients with Eustachian tube dysfunction.

08
Overlay & Underlay Techniques

Underlay: Graft is placed beneath the eardrum remnant; widely used, reliable
results.

Why You Should Not Delay

Delaying treatment for a perforated eardrum can lead to persistent hearing loss, repeated infections, and long-term ear damage. Early surgery ensures better hearing outcomes, fewer infections, and improved quality of life.
